Insurance
Vision Insurance. Vision insurance covers routine
eye examinations, eyeglasses or contact lenses. The insurance benefit
if usually limited to one visit per year. Eye problems covered by vision
insurance include nearsightedness, farsightedness, and astigmatism.
We accept many insurance plans at this office, including Tricare, Vision
Service Plan, Scott & White Health Plan, Blue Cross Blue Shield
of Texas, Medicare, and Medicaid. As a participating provider, we will
file claims for you and wait for payment from the insurance company.
Please ask a staff member for assistance if you plan to use vision insurance
to cover some or all of the fees for your visit. If you have vision
insurance and we are not a participating provider on that plan, you
will receive a copy of your itemized charges so that you may file your
claim.
Health Insurance. Health insurance covers office visits
to diagnose or treat a medical eye problem. Return visits are covered
as long as they are medically necessary. Eye conditions covered by health
insurance include glaucoma, corneal abraisions, ocular infections, dry
eyes, eyelid problems, diabetic eye disease, cataracts, and ocular allergies.
We accept some of the health plans in the area. If you plan to use health
insurance to cover some or all of the fees for your visit - especially
if you belong to an HMO - please ask a member of the staff for assistance.
Medicare. We are participating providers under the
Medicare Part B program. As participating providers, we file claims
for you and accept what Medicare considers a fair fee for services rendered.
Medicare covers 80% of what is considered a fair fee. You are responsible
for your Medicare deductible as well as the remaining 20% co-payment.
Supplemental insurance or Medicaid often covers the remaining 20% balance.
Please notify the staff if you have Medicare replacement HMO insurance.
Medicaid. If you have Medicaid insurance coverage,
please bring your up-to-date Medicaid eligibility letter with you for
every visit. If you have insurance in addition to Medicaid, please give
the receptionist all other insurance information as well. If referral
numbers or authorization numbers are needed to file a claim, this information
must be obtained prior to or at the beginning of the patient's visit.
